Who we are
Aldrich Health Services is a Minnesota DHS-licensed 245D Home and Community-Based Services (HCBS) provider serving individuals across the Twin Cities metro area. We partner with individuals, families, and case managers to deliver person-centered, culturally responsive supports that promote independence, dignity, and meaningful progress toward individual goals.
Our organization is guided by a leadership team with over 20 years of combined experience across community-based services, healthcare, and corporate leadership. This depth of experience allows us to pair compassionate, hands-on support with strong operational practices, clear communication, and dependable service delivery.
Our Approach
We believe that a diagnosis, disability, or life challenge does not define a person. Our work begins with building trust and strong relationships, taking time to understand each individual’s strengths, preferences, culture, and goals. This foundation allows us to accurately assess support needs and deliver services that are truly aligned with the person’s desired outcomes.
We work collaboratively with case managers and interdisciplinary teams, valuing clear communication, responsiveness, and shared accountability. Our approach is grounded in culturally responsive, person-centered care that respects each individual’s background, lived experience, and right to direct their own life.
